Welcome to the first newsletter for the Woodbury, Exmouth & Budleigh PCN

The WEB PCN is made up of 6 practices across the Woodbury, Exmouth & Budleigh Salterton area delivering primary care services to a population of over 51,000. Our PCN practice members are Budleigh Salterton Medical Centre, Claremont Medical Practice (incorporating Raleigh Surgery), Haldon House Surgery, Imperial Surgery, Rolle Medical Partnership and Woodbury Surgery.

Be Kind

Over the last 18 months, our practices have been supporting the pandemic response and ensuring our patients are able to get their COVID-19 vaccinations, at the same time as continuing to deliver primary care services.

This has made our GP practices very busy, and our staff have been working incredibly hard to keep services open for patients. We’re aware patients may be experiencing longer wait times for GP appointments and prescriptions and may be experiencing significant delays for a hospital appointment. Our practice staff are working hard to ensure you receive the right treatment from the right person at the right time. However, as staff shortages continue across all practices and businesses there can be delays in our systems.

We understand things may be difficult for our patients currently, but we ask you to try and be kind with our staff and colleagues as we all continue to support you.

The NHS is still here to help. There is free advice available 24 hours-a-day on both the nhs.uk and 111.nhs.uk websites. Local pharmacists are also able to help with a wide variety of common health issues. So please, if you contact our practices, be kind.

 

Enhanced care in care homes

Our key focus of this project is to help make Care Homes a place where residents and staff alike can thrive.

The service provides bespoke individual care plans to maintain good health and maximise quality of life; it develops better links between GP Practices and Care Homes to enhance medical care and easy access to Therapy Teams (e.g. Physiotherapy); and it works with community and voluntary services to bring activities and interests to Care Homes to enrich the residents lives.

 

Pharmacists

Our Clinical Pharmacy Team provide medication reviews for all WEB patients to ensure they remain the best option for the individual.

They also work within our local Care Homes as part of our Enhanced Health Care in Care Homes team.

Our pharmacists have been instrumental in the successful delivery of more than 70,000 doses of Covid Vaccine to the Communities of East Devon.

 

Extended scope physiotherapists (ESPs)

Keeping the communities in WEB fit and active is vitally important for our health and wellbeing. Our team of highly skilled Extended Scope Physiotherapists (ESPs) are on hand every day of the week to help you achieve this.

If you have a new or recurring injury, please contact your GP practice to discuss a referral to one of our team to have this injury expertly assessed, investigated and treated. If your injury requires the skills of our local orthopaedic team, thanks to our links with the Royal Devon & Exeter NHS Foundation Trust, the ESPs will be able to refer you directly to the Consultant.

 

Children and young people

Young Devon are commissioned to provide counselling and wellbeing support to students in Exmouth Community College. Wellbeing practitioners are based in the college and able to offer face to face and virtual  individual CBT sessions, wellbeing conversations and counselling support.

A young person or parent can access support directly via the Young Devon website or alternatively an ECC student can speak to their Pastoral Support Assistant who will help them.

We have partnered with HeadsUp to provide mental health support to students within the WEB schools and colleges.

HeadsUp offer free sessions to get young people confident in talking about mental health.

Coming soon

We are excited to be launching a new Primary Care Mental Health & Wellbeing Team in March 2022.

The team will consist of a Mental Health Link Worker, Community Connectors and Health & Wellbeing Coaches who will have a combined aim of supporting people with their mental health and wellbeing by providing a holistic service that will help individuals in identifying the barriers to their good mental health.

The service will be taking referrals for mild to moderate mental health issues from all the WEB PCN practices and will offer a range of services including mental health & wellbeing coaching sessions, signposting and onward referral to other services, support in connecting with community assets and confidence building support.

 

Outstanding person-centred care award

This season we had some amazing nominations for this award which focuses on our care home staff. We are very proud to announce three winners for the Winter 2022 Outstanding Person-Centred Care Award.

These are Donna Fiddy at Halsdown (pictured right), Sharon Kyte at Raleigh Manor (below left) and Rachel Callaghan at Pinewood (below right). Huge congratulations to them all!

Have fun, get fit!

Join the first ever Woodbury, Exmouth and Budleigh Care Homes Team taking part in the Alzheimers Society Trek26 Event on 10th September 2022. Choose to walk 13 miles or 26 miles, through beautiful Devon countryside, in a circular route starting at Brixham.

If you are new to trekking, we will be with you every step of the way, and there will be optional training walks to help you prepare! This is a great opportunity to make new friends and fundraise together, all are welcome.
